Доказанным условием предотвращения прогрессирования глаукомной оптической нейропатии является снижение внутриглазного давления. Терапию начинают с местных гипотензивных препаратов, при их недостаточной эффективности проводят лазерное лечение или прибегают к хирургии. Однако проведение гипотензивного вмешательства не гарантирует достижения стойкого гипотензивного эффекта. Заживление ран является сложным физиологическим динамическим процессом, который необходим для поддержания гомеостаза в организме. В этом процессе выделяют три связанные между собою фазы: воспаление, фиброплазию и ремодуляцию (созревание) рубца. В обзоре представлены различные клеточные механизмы, участвующие в регуляции данных процессов и способствующие избыточному рубцеванию. Современные знания о ключевых факторах патоморфологических процессов, происходящих в области фильтрационной зоны, призваны стимулировать разработку новых методов пролонгации действия фильтрующей хирургии.</p></abstract><trans-abstract xml:lang="en"><p>Glaucoma is a leading cause of blindness in the world. A proven condition for preventing the progression of glaucomatous optic neuropathy is to reduce intraocular pressure. Therapy begins with local hypotensive drugs; if they are ineffective, laser treatment or surgery is used. However, hypotensive intervention does not guarantee a stable hypotensive effect. Wound healing is a complex physiological dynamic process that is necessary to maintain homeostasis in the body. This process includes three interrelated phases: inflammation, fibroplasia, and remodulation (maturation) of the scar. Thе review presents various cellular mechanisms involved in the regulation of these processes and factors that contribute to excessive scarring.
