Материал и методы. В исследование включены 64 пациента (64 глаза) в возрасте 52 ± 10 лет с аметропиями различных степеней и видов, а также с катарактой I–III степени плотности ядра хрусталика по классификации по PNS и Буратто. Предоперационная денситометрия проводилась на Pentacam и Galilei G6. Данные использовались для персонализированного подбора энергии фемтолазера и паттерна фрагментации ядра. Пациенты 1-й группы (n = 33) получали комбинированный препарат (нестероидное противовоспалительное средство (НПВС) и антибиотик), во 2-й группе (n = 31) — раздельно НПВС и антибиотик. Результаты. Персонализированные параметры фемтолазера обеспечили эффективную фрагментацию ядра во всех случаях. Комбинированная терапия показала преимущества перед раздельной в отношении стабильности мидриаза, профилактики воспаления и приверженности лечению. Заключение. Исследование подтверждает важность денситометрической оценки плотности хрусталика и комбинированной медикаментозной терапии для повышения эффективности и безопасности рефракционной хирургии хрусталика с фемтолазерным сопровождением.</p></abstract><trans-abstract xml:lang="en"><p>Purpose the study — to evaluate the effectiveness of a personalized approach to femtosecond laser fragmentation of the lens nucleus based on densitometry, as well as to analyze the effect of a combination drug (ketorolac + levofloxacin) on perioperative patient management. Material and methods. The study included 64 patients (64 eyes) aged 52 ± 10 with ametropia of various degrees and types, as well as with cataracts of I–III degrees of lens nucleus density according to the PNS and Buratto classification. Preoperative densitometry was performed on Pentacam and Galilei G6. The data were used for personalized selection of femtosecond laser energy and nuclear fragmentation pattern. Patients of group 1 (n = 33) received a combination drug (non-steroidal anti-inflammatory drug (NSAID) and antibiotic), in group 2 (n = 31) — NSAID and antibiotic separately. Results. Personalized femtolaser parameters provided effective fragmentation of the nucleus in all cases. Combined therapy showed advantages over separate therapy in terms of mydriasis stability, inflammation prevention, and treatment adherence. Conclusion.
