Материал и методы. Проведено сравнение биомеханических показателей 45 глаз 23 пациентов с СПД (26 глаз) и ПГ (19 глаз) в возрасте 33,6 ± 1,5 года. На 5 глазах пациентов этой группы было проведено кераторефракционное вмешательство («ФемтоСуперЛАСИК»). На 34 глазах с СПД и ПГ была произведена ПИЭ. Обследовано также 30 глаз с миопией без СПД и ПГ в возрасте 40,2 ± 1,8 года (группа сравнения) и 18 глаз пациентов с эмметропией без глаукомы в возрасте 37,00 ± 1,93 года (группа контроля). Биомеханические показатели и биомеханически скорректированное ВГД (bIOP) определяли с помощью Pentacam (Oculus) и CorVis ST. Результаты. Глаза с СПД и ПГ достоверно не отличались по биомеханическим параметрам от миопических глаз без СПД и ПГ. В то же время биомеханический глаукомный фактор (BGF) при СПД и ПГ был ниже, чем при миопии (р = 0,025 и р = 0,015), что косвенно характеризует фиброзную оболочку глаз с СПД и ПГ как более ригидную. SSI (показатель внутренней жесткости роговицы) глаз с СПД, ПГ и миопией оказался сниженным в сравнении со здоровыми эмметропичными глазами (р = 0,000, р = 0,019, р = 0,005 соответственно). После проведения ПИЭ уровень bIOP снизился (р = 0,015), изменились значения характеристик жесткости роговицы — увеличился Integr. Radius (р = 0,002) и снизился показатель SP-A1 (р = 0,022), что говорит о снижении жесткости ткани роговицы. При этом BGF увеличился (р = 0,026) и достиг соответствующих значений близоруких глаз без СПД и ПГ. Заключение. С учетом сниженного индекса SSI кераторефракционное вмешательство при СПД и ПГ следует проводить с осторожностью. ПИЭ при СПД и ПГ можно рекомендовать не только для снижения ВГД, но также для нормализации показателей биомеханики и приближении их значений к параметрам близорукого глаза без СПД и ПГ.</p></abstract><trans-abstract xml:lang="en"><p>Purpose of the work was to study the features of biomechanical properties of the fibrous capsule of the eye in pigment dispersion syndrome (PDS) and pigment glaucoma (PG), and to evaluate the effect of peripheral iridectomy (PIE) in these conditions on the biomechanics of the eye. Material and methods. Biomechanical parameters of 45 eyes of 23 patients with PDS (26 eyes) and PG (19 eyes) aged 33.6 ± 1.5 years were compared. Keratorefractive surgery (Femto Super LASIK) was performed on 5 eyes of patients in this group. PIE was performed on 34 eyes with PDS and PG. The examination also included 30 eyes with myopia without PDS and PG aged 40.2 ± 1.8 years (comparison group) and 18 eyes of patients with emmetropia without glaucoma aged 37.00 ± 1.93 years (control group). Biomechanical parameters and biomechanically corrected IOP (bIOP) were determined using Pentacam (Oculus) and CorVis ST. Results. Eyes with SPD and PG did not significantly differ in biomechanical parameters from myopic eyes without SPD and PG. At the same time, the biomechanical glaucoma factor (BGF) in SPD and PG was lower than in myopia (p = 0.025 and p = 0.015), which indirectly characterizes the fibrous membrane of the eyes with SPD and PG as more rigid. SSI (an indicator of internal corneal rigidity) of the eyes with SPD, PG and myopia was reduced in comparison with healthy emmetropic eyes (p = 0.000, p = 0.019, p = 0.005, respectively). After the PIE, the bIOP level decreased (p = 0.015), the values of corneal rigidity characteristics changed: Integr. Radius increased (p = 0.002) and the SP-A1 index decreased (p = 0.022), which indicates a decrease in corneal tissue rigidity. At the same time, BGF increased (p = 0.026) and reached the corresponding values of myopic eyes without SPD and PG. Conclusion. Taking into account the reduced SSI index, keratorefractive intervention in SPD and PG should be performed with caution.
