Значимость восстановления прозрачности роговицы и повышения остроты зрения превалирует над хорошо известными недостатками этих методик. В качестве альтернативы при изолированной и преимущественной патологии стромы роговицы нами ранее была предложена новая разновидность закрытой кератопластики — оптическая селективная трансплантация стромы (оСТС). Методика оСТС лишена внутриглазных осложнений, сквозной раны, швов, сохраняет интактными поверхности роговицы, исключает осложнения, связанные с эпителизацией, оптимизирует рациональное использование доступной донорской ткани роговицы. Однако, как показало время, этап резекции стромы при оСТС оказался технически сложным для рутинного и широкого применения.</p><p>Цель работы — представить модифицированную (упрощенную) хирургическую технику селективной трансплантации стромы (мСТС).</p><sec><title>Материал и методы</title><p>Материал и методы. мСТС выполнена у 4 пациентов (средний возраст — 48 ± 10 лет) с решетчатой дистрофией роговицы. Период наблюдения составил 8,0 ± 1,3 мес. До операции и в фиксированные сроки после проводились рефрактометрия, визометрия, биомикроскопия, кератоанализирование, оптическая когерентная томография роговицы. Оценивали необходимость использования режущих инструментов и продолжительность этапа экстракции резицированной стромы роговицы реципиента при оСТС и мСТС.</p></sec><sec><title>Результаты</title><p>Результаты. Средняя продолжительность этапа экстракции при мСТС составила 336,0 ± 43,7 с, в то время как при оСТС — 838,0 ± 233,5 с. Дополнительно режущий инструмент при мСТС не использовался ни в одном случае против 75 % случаев в исходной оригинальной методике.</p></sec><sec><title>Заключение</title><p>Заключение. В мСТС продолжительность наиболее сложного этапа, а именно этапа экстракции стромы роговицы реципиента, существенно сокращена, что потенциально делает методику рутинной и более доступной широкому кругу офтальмохирургов.</p></sec></abstract><trans-abstract xml:lang="en"><p>Deep anterior lamellar keratoplasty and penetrating keratoplasty (PKP) are the operations of first choice for pathological changes of the cornea localized in the stroma or incurable pathology of the corneal stroma. The importance of restoring corneal transparency and increasing visual acuity prevail over the well-known disadvantages of these techniques. In case of isolated corneal stroma pathology we have previously proposed a new type of closed keratoplasty – selective optical transplantation of the stroma (oSTS). The oSTS technique is devoid of intraocular complications, penetrating wound, sutures, keeps corneal surfaces intact, excludes complications related to epithelialization, optimizes rational use of available donor corneal tissue. However, as time has shown, the stromal resection stage in oSTS turned out to be technically complicated for routine and wide application.</p><sec><title>Purpose of the study</title><p>Purpose of the study: to develop a modified (simplified) surgical technique of oSTS.</p></sec><sec><title>Material and methods</title><p>Material and methods. Modified STS (mSTS) was performed in 4 patients (age 48 ± 10 years) with lattice corneal dystrophy. The follow-up period amounted to 8.0 ± 1.3 months. Refractometry, visometry, biomicroscopy, keratoanalysis, optical coherence tomography of the cornea were performed before the operation and at fixed periods after it. We evaluated the necessity for the use of cutting instruments and the duration of extraction step of the recipient’s resected corneal stroma in both at original oSTS and mSTS.</p></sec><sec><title>Results</title><p>Results. The mean duration of the extraction step in mSTS was 336.0 ± 43.7 s, whereas in oSTS it was 838.0 ± 233.5 s. No additional cutting instrument was used in the mSTS, compared to 75 % of patients in the oSTS.</p></sec><sec><title>Conclusion</title><p>Conclusion.
