Обследовано 154 глаза 136 пациентов, получивших боевую закрытую травму заднего сегмента глаза различной степени тяжести, с помощью стандартных офтальмологических методов, а также оптической когерентной томографии (ОКТ) и ОКТ-ангиографии. Выявлены и отобраны для анализа пациенты с ОМН, для которых оценивались структурные изменения сетчатки на кросс-секционных сканах, а также площадь зон ОМН на структурных анфас-изображениях.</p></sec><sec><title>Результаты</title><p>Результаты. ОМН, характеризующаяся истончением наружной сетчатки, выявлена на 10 глазах 10 пациентов (10 мужчин, средний возраст — 29 ± 4 года), что составило, таким образом, 6,5 % случаев от всех обследованных глаз с закрытой травмой. Анализ кумулятивного распределения показал тренд к локализации участков ОМН преимущественно с назальной стороны от центра макулы со средним значением площади зоны ОМН 2,4 ± 1,4 мм2. ОМН являлась изолированным проявлением травмы только в 2 случаях, в остальных комбинировалась с другими проявлениями закрытой травмы глаза.</p></sec><sec><title>Заключение</title><p>Заключение. ОМН — одно из типовых проявлений закрытой травмы глаза с распространенностью около 6,5 % среди комбатантов с закрытой травмой глаза. ОМН может являться причиной стойких изменений зрения, чаще в виде парацентральных и центральных скотом, и объяснять «необъяснимые скотомы» и «необъяснимое снижение зрения» у пациентов с относительно легкой травмой. Вклад ОМН в структуру закрытой травмы глаза хорошо укладывается в концепцию синдромального характера этого феномена. Диагностика ОМН требует применения ОКТ даже в ситуациях отсутствия офтальмоскопических изменений в центре макулы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: to analyze the prevalence and clinical significance of acute macular neuroretinopathy (AMN) in combat closed globe trauma using multimodal imaging.</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, 154 eyes of 136 patienys with combat closed globe trauma of the posterior segment received standard ophthalmological examination and optical coherence tomography (OCT) and OCT angiography. Patients with AMN were identified and selected for analysis. Structural changes of the retina on cross-sectional scans, as well as the area of AMN lesions were analyzed on structural en-face projections.</p></sec><sec><title>Results</title><p>Results. AMN characterized by thinning of the outer retina was detected in ten eyes of ten patients (10 males, mean age 29 ± 4 years), thus accounting for 6.5 % of cases from all examined eyes with closed eyeball trauma. The analysis of the cumulative distribution showed a trend towards localization of the AMN lesions mainly in the nasal sector of the macula with an average area of 2.4 ± 1.4 mm2. AMN was an isolated manifestation of injury only in two cases, in eight eyes it was combined with other injuries associated with closed eyeball trauma.</p></sec><sec><title>Conclusion</title><p>Conclusion. AMN is one of the typical lesions in closed eye injury, with a prevalence of 6.5 % among these cases. AMN can cause persistent visual deterioration, including paracentral and central scotomas and may explain “unexplained scotomas” and “unexplained decrease of vision” in patients with mild injury. The contribution of AMN to the structure of closed globe injury agrees with the concept of the syndromic nature of this phenomenon.
